Science News: Low Sensitivity of Skin Biopsy in Diagnosing Small Fiber Neuropathy in Chinese Americans

Small fiber sensory neuropathy (SFSN) is a common referral for NM and EDX medicine. The use of skin biopsy to measure the intraepidermal nerve fiber density (IENFD) is widely accepted as a standard diagnostic test for SFSN; however, the normative values have not been fully established in diverse patient populations. This article is important in practice because it provides evidence that IENFD may vary in different patient populations. This information will affect the diagnosis, management and outcomes if ethnic differences are not accounted for in the interpretation of data.

CIDP is one of the most common chronic demyelinating polyneuropathies encountered in clinical practice. Recently recognized atypical variants can be missed at onset and constitute around 20% of the cases at presentation. Recognizing them at the onset and treating as typical CIDP is important as they cause similar disability and delay in treatment may lead to permanent damage.
