Office and Outpatient Evaluation and Management Coding and Documentation Guidelines

Accurate evaluation and management (E/M) coding and billing are essential for maintaining the efficiency and productivity of medical practices. E/M coding utilizes CPT codes between 99202 and 99499, which cover services where a provider assesses or oversees a patient’s health. However, procedures like diagnostic tests, radiology, surgeries, and specific treatments do not fall under E/M services. The AMA strives to streamline E/M codes and alleviate burdens on physicians.
