This is a randomized, double-blind control trial of autologous platelet rich plasma (PRP) in adults with chronic (≥ 6 months), moderate-to-severe lumbar discogenic pain, unresponsive to conservative treatment. Forty-seven participants (29 in the treatment group, 18 in the control group) were analyzed by an independent observer, and at eight weeks there were statistically significant improvements in pain, functional outcome, and overall satisfaction in participants who received intradiscal PRP. No adverse events, including disc space infection, neurologic injury, or progressive herniation were reported following PRP injection. Those who received PRP maintained significant improvements in functional rating scores scores through at least one year of follow-up. 

Comment: Chronic lower back pain is a widespread problem, and available treatments have not proved effective. PRP injections are employed for other musculoskeletal conditions, and this study shows promising early results. Further studies are needed to define the subset of participants most likely to respond to biologic intradiscal treatment and the ideal cellular characteristics of the intradiscal PRP injectate.
